2. Bulk food scoop was stored so handle was buried in the food.
3. A bag of onions was stored directly on floor.
Corrective Action(s): 1. Cover all containers of food to protect them from potential contamination.
2. Store food scoops in a manner that prevents the handle from contacting food. Food scoops may be stored with handle pointing upright or in a dedicated storage container. Ensure scoops and storage container (if used) are cleaned and sanitized regularly.
3. Store bag of onions off the floor.
