Observation (CORRECTED DURING INSPECTION): Concentration of sanitizer in final rinse of dishwasher tested at 0ppm chlorine. Sanitizer container containing chlorine sanitizer had oxidized resulting in no concentration being detected in the final rinse. Once a new container of sanitizer was installed and the dishwasher primed a final rinse concentration of approx. 100ppm was detected in the final rinse. This is a REPEAT VIOLATION which had been documented in previous inspection reports.
Corrective Action(s): Rewash all dishware in the premises
Ensure the employee who is entrusted to document the dishwasher concentration has been properly trained.

Observation: Loose green poison observed on the floor below shelving in dry food storage room
Corrective Action(s): Remove all poison as poison is not premitted for use inside a food service establishment. Only traps may be used ie snap traps, trap boxes or glue boards. The pest control company will supply these.,

Observation: Mouse droppings observed both in the back dry food storage room and in the up front sushi prep area,.
Corrective Action(s): Hire a pest control company
Remove all old droppings ie behind hot water tank, below and behind shelving in dry food storage room.
Purchase bins for the front sushi containers and bin those until the pest problem has been eliminated

Observation: Toilet observed to be not flushing properly in the handicapped washrrom. This is an outstanding issue from report on June 26, 2023

Employee falsifying dishwasher records. Ensure employees have been properly trained on how to test the sanitizer concentration at the dishwasher.
Premises will continue to be placed on ELEVATED INSPECTIONS due to repeat CRITICAL VIOLATIONS.
All cold holding equipment cold holding at <4c
Miso soup hot holding at >60c
Wiping rags observed in containers containing bleach at a concentration of 100ppm. Recommend using Quat as a sanitizer at 200ppm as this does not impart and odor issues.
Tags observed on all hoses for the dishwasher
Timers in place at deep fryers which alarm once the cooking cycle has been completed for the chicken balls and the teriyaki chicken
