Observation (CORRECTED DURING INSPECTION): 1) Sanitizer pail only had 50 ppm QUATs at the time of inspection. Operator discarded the sanitizer and resupplied sanitizer pail with 200 ppm QUATs.
2) Operator was observed not following proper manual dishware washing practices. Operator was reminded of proper manual dishware washing practicies.

Corrective Action(s): - Remove and discard all ice in machine

- Do not use Ice produced from this ice machine until it has been cleaned and sanitized
- Start and maintain a cleaning log for cleaning and sanitizing the ice machine
- This is a REPEAT VIOLATION
