Observation (CORRECTED DURING INSPECTION): No approved food contact sanitizer available on site at this time. 100 ppm chlorine sanitizer prepared by adding 1 teaspoon bleach to 1L of water.

Observation (CORRECTED DURING INSPECTION): Both of hand washing stations' paper towel dispensers were empty. Paper towels made available at this time.

Observation: Observed the following foods not protected from contamination:
- Open bags of pre-dip batter powders
- Container of sugar with out a lid
- Open bags of ground coffee
Corrective Action(s): Products described in the observation section to be discarded due to possible biological contamination.

304 - Premises not free of pests [s. 26(a)]
Observation: Observed live cockroaches in the following areas:
- Under the 3 compartment sink on top of the plumbing (30-40 live roaches)
- Behind ice cream freezer (1 live roach)
- Inside of cabinet door named 'kitchen ware' (3-4 live roaches)
- Under the wooden table on the hall way (1 live roach)

Cockroach droppings observed in the following areas:
- Inside a drawer labelled as 'Hot-cup lids' and 'small containers lid'
- Behind prep cooler work station
- Men staff washroom
- Mop sink room
- Under the 3 compartment sink
- Inside of cabinet door named 'kitchen ware'
- On top shelf of prep cooler work station where take out containers are stored.
- Throughout the facility, especially wall to floor joints.

Dead cockroaches found throughout the facility.
Corrective Action(s): Contact a pest control company for an assessment. Clean and sanitize all areas impacted by cockroach activities. Adhere to your pest control company's recommendations to control the cockroach activity. Correct this by: immediately.

Observation: 1. Observed flattened cardboard boxes stored on the hall way to the washroom.
2. Number of items that are not used for day-to-day operations stored on site (e.g., 2 door cooler, utensils that are not used, etc.)
3. Heavy clutter in the room next to the back door.
4. Heavy oil/grease build up on the floor by the deep fryer.
5. Food debris under the deep fryer.
6. Holes on the wall under the 3 compartment sink.
7. Hole on the back door glass door.
Corrective Action(s): 1-3. Remove items that are not required for day-to-day operation to reduce clutter and harboring sites.
4-5. Sanitation improvement required to discourage cockroach activity.
6-7. Block any holes or crevices on the premise to stop cockroaches from entering.
