Observation: Improper dispensers, single-use cups, used and stored within bulk food containers.
Corrective Action(s): Remove all single-use cups and replace with proper dispensers with handles. Store in a manner where the handles do not come into contact with food.

Observation: 2 live cockroaches were noted during the inspection. No other pest activities were noted.
Corrective Action(s): As per staff, they have a pest control company that comes in monthly. Ensure all food is covered. Maintain the sanitary condition of the facility and continue to work with your pest control company to address and mitigate the pest issue.

Observation: 1. < 10 PPM chlorine residual was detected from the low temperature dish machine. As per staff, they have chlorine test strips available onsit and the machine was working properly the day before. The restaurant just opened when the inspection started and the staff knew there was a problem when the machine was being set-up.
2. Taps for the handwash station in the kitchen area were noted loose.
Corrective Action(s): 1. **Issue corrected at the time of inspection** The low temperature dish machine was immediately serviced. After serving, 100 PPM chlorine residual was detected after the rinse cycle. No dishwares were being washed while the dish machine was out of service.
2. Fix the taps so water is there is no leaking water when the faucet is turned off.

Observation: Operator has renewed his FOODSAFE certification however he was not present during the time of inspection. Staff has an expired FOODSAFE certification.
Corrective Action(s): This is the final warning, staff must renew his FOODSAFE level 1 or equivalent certificate. Operator must ensure that at least one staff has a valid FOODSAFE certificate when he is not at the facility. Operator must email the covering district inspector the staff's completed certification within the next 2 months (2 months granted due to language barrier).
