Observation: 1. Whole raw eggs observed stored above cooked pasta in walk-in cooler.
2. Cheese grater, stored clean, observed with a build-up of rust on blades.
3. Unknown type of yellow glue used to seal cracks in plastic insert.
Corrective Action(s): To prevent biological, physical, and chemical contamination:
1. Raw eggs are to be stored below all ready to eat foods.
2. Equipment containing rust is to be discarded or replaced.
3. Only food safe materials are to be used to store foods. Discard or replace any cracked equipment, containers, or utensils. Glues are not to be used to repair food containers.

Observation (CORRECTED DURING INSPECTION): 1. Stored clean cheese grater observed with rust on the blades.
2. Stored clean plastic insert observed with cracks that were sealed with unknown type of yellow glue.
Corrective Action(s): Food contact surfaces were discarded. Ensure all food contact surfaces are durable, smooth, and free from toxic materials (rust, glue).

Observation: Pizza hot holding unit not maintaining a temperature of 60 C or higher. Ambient temperature observed at 43.3 C during inspection. No single slice pizza being offered at this time.
Corrective Action(s): Do not use hot holding unit. Have unit repaired or replaced with a unit that can maintain a temperature of 60 C or higher.

Observation: 1. Damaged floor tile observed in front kitchen area by pizza oven.
2. Damaged baseboard observed in preparation area.
3. Large holes observed in wall in staff bathroom.
Corrective Action(s): To maintain the premises in sanitary condition, all materials are to be durable, smooth, and easy to clean. Repair or replace damaged items to maintain premises as approved plans. Date to be corrected by: June 15, 2022.

- Coolers achieving temperatures of less than or equal to 4 degrees Celsius
- Freezers achieving temperatures of less than or equal to -18 degrees Celsius
- Chlorine sanitizer solution in use throughout food preparation areas and at concentrations of greater than or equal to 200ppm
- Two functional sink plugs available for use observed
- Hair restraints in use
- Back door closed during inspection
- Pest proof containers for all dry storage items
- Scoops placed external to dry storage bins
- Light covers in place
- Cooler handles and seals in good sanitary condition
- Staff Personal items kept separate from kitchen equipment
- FoodSafe Certified staff present during inspection

NOTE: Ensure all cooked foods are cooled from 60 degrees Celsius to 20 degrees Celsius within 2 hours, and from 20 degrees Celsius to 4 degrees Celsius within 4 hours. Foods are to be cooled as follows:
1. Submerged in ice water bath
2. Stirred with ice wand
3. Placed in shallow container in refrigeration at 4 degrees Celsius or lower.
