Observation (CORRECTED DURING INSPECTION): 1. 2-door prep cooler #1 is too warm, at 10.6oC air temperature. Temperature log indicated temperature of 3oC at 11am this morning (now 3pm). Food internal tempertures are measured at 5.9oC (cooked onions) and 6.3oC (cooked tomatoes).
2. Garlic sauce in squeeze bottle is observed storing at front at room temperature for customers.
Corrective Action(s): 1. Do not store any potentially hazardous foods in cooler until temperature is maintained at 4oC or less. Staff relocated all foods to another cooler.
2. Garlic sauce must be stored in cooler under 4oC, unless lab result is provided indicating sauce can be held at room temperature.

Observation (CORRECTED DURING INSPECTION): 1. Discussed manual dishwashing procedures with staff - Quats sanitizer is used to sanitize dishware. Having said that, it is indicated dishware is rinsed after sanitizing, before air drying.
2. 0ppm noted in water inside sanitizer sink.
Corrective Action(s): 1. Do not rinse dishware after sanitizing: wash in soapy water; rinse in clean water; soak in sanitizer; air dry.
2. Purchase Quats test strips to test sanitizer concentration.

Observation: Products observed storing on ground throughout facility - oil cartons; food buckets in walk-in cooler and food preparation area; and unused pizza cardboard.
Corrective Action(s): Products, both food and non-food, must be stored at least 6 inches above ground to faciltate cleaning and for pest monitoring.
Do not use pop crates as means to store products above ground.

Observation: Rodent droppings observed under shelving rack next to office station; on floor behind dough prep table; on floor next to beverage cooler; on shelves under POS station. Pest control company visits twice montly. Pest control reports cannot be provided at time of visit.
Corrective Action(s): 1. Provide pest control reports for review next inspection.
2. Remove droppings, sanitize areas, and monitor.
3. Seal all entry points --> a thorough inspection should be done by pest control company.

Observation: Holes observed on ceiling tiles. One hole above hot water tank observed with cardboard and tape.
Corrective Action(s): Seal hole or replace ceiling tiles. Use proper materials to deter pests from entering the facility. Cardboard will not deter pests.

Observation: 2-door prep cooler is too warm, at 10.6oC.
Corrective Action(s): Do not use cooler until temperature is back down to 4oC or less. Repair cooler if necessary. Temperature of cooler is slowly coming down, at 7.3oC at the end of inspection. Keep monitoring cooler.
