Observation:
1) Hole in the wall observed by the scale in the hallway.
2) Sliding glass door on chest freezer has not been repaired as per previous inspection; food debris build up observed in the tape.
3) Walls in walk-in cooler are peeling; food debris build up present in cracks.
Corrective Action(s):
1) Seal all holes in the facility. Ensure that these surfaces are smooth, waterproof and easy to clean.
2) Repair glass door.
3) Repair this surface; surfaces must be smooth, waterproof and easy to clean.
