Front of house handwashing station observed to be obstructed with bucket of bleach solution. Handwashing station must always remain unobstructed to allow staff to wash their hands. Bucket has been removed to be stored next to handwashing station. 2. Back handwashing station observed lacking hand soap. Handwashing station must always be fully equipped with hot and cold running water, soap, and paper towels. Soap has been replaced. Corrective Action(s): Violation Score: 5

- Staff has FOODSAFE. ***Operator is in the process of getting his FOODSAFE. - Rodent droppings has been cleaned up. ***Please continue to monitor pest activities. - Ventilation hood has been cleaned. - Fans in walk in cooler has been cleaned - Back door is now closed and a sign is placed to remind staff to keep the back door closed. - Food has be reorganized to ensure raw food is placed under cooked and ready to eat foods. - No more trays of food is observed on the ground. - Ice machine is now cleaned. - Temperature log is now present. **Bucket of water under the dishwasher is still present. Please ensure a technician service the dishwasher to stop the leak. **Bucket of water under the back sink by the back door. Please fix the leak, remove the bucket of water, and clean the area.
