Observation (CORRECTED DURING INSPECTION): Multiple plastic doors for the sushi bar cooler were removed. Operator stated this was done during the rush hours for easy access. Internal food product temperatures measured at 7C.
Corrective Action(s): Do not remove cooler doors (covers) at any time. Cooler can not maintain an ambient temperature of 4C or less when door is removed. Staff installed the door back.

Observation (CORRECTED DURING INSPECTION): Containers raw meats stored on top / beside vegetables in multiple coolers and freezers.
Corrective Action(s): Do not store raw meats on top / beside any fresh and ready to eat foods. Store raw meats on the bottom shelf in the coolers and freezers, away from other food.

Corrective Action(s): Clean up the rodent droppings and continue monitor for any pest activities. Correct by: today.

Observation (CORRECTED DURING INSPECTION): Back kitchen entrance door was open. No delivery was being made at the time.
Corrective Action(s): Close the back kitchen entrance door to prevent pest entry. Ensure the door is closed at all times except during deliveries and emergencies.
