Observation: Permit is expired on March 31, 2018.
The outstanding permit fee letter has been delivered on 07/17/2018.

Observation: Mice droppings found on storage shelves, in the staff washroom, under the front hand sink/in the cabinet, & between equipment.
Corrective Action(s): Today. Clean and sanitize the entire kitchen.

Observation: General sanitation required improvement. Noted accumulation of grease on a fan hood (the due day for cleaning on Jan 5, 2019); Grease on the floor under the 2-compartment sink; food debris on storage shelves; white mold & debris on shelves in the walk-in cooler; debris on the top of the dishwasher; dirty walls under prep. tables;
Corrective Action(s): Today. And ongoing. Call to the service company to clean the fan hood.
Clean (with hot water and soap) and sanitize (with Bleach solution/100 ppm) all shelves;
Clean & sanitize the floor; top of the dishwasher; & walls.

Comments

Front hand washing stations supplied with hot/cold running water, soap, and paper towels. Install soap dispenser at the back hand-washing station in two weeks.
Coolers (2 upright coolers, walk in) at < or = to 4 C.
Hot holding unit (soups) at 60 C. Rice cookers measured at 60 C.
High temperature dishwasher reached 73 C at the plate's level during the final rinse cycle.
Dry storage area has sufficient cabinet space and wire racks.
Pest proof containers for dry food storage available with proper lids.

Note: The manager on site stated that a pest service on site once a month.

Note: Prep. cooler bit high (8.1 C) - ensure, the temperature of the prep. cooler < = 4.0 C;
Monitor the temperature and keep temperature log.
The inspection is conducted during the lunch time.

Recommendation:
1. Labeled spray bottles with sanitizer must be available all the time.
2. All food must be properly covered.
3. Missing floor-wall junctions must be replaced.
4. Food must be cooled and thawed properly.
5. Replace broken plastic measurement container for rice.
6. Equipment that is not in used must be clean/or removed from the premise.
7. Sanitation plan must be submitted to the Health Department (as per the previous reports).

Note: Permit Fee must be paid (250$)/Due day- 01.11.19.
Enforcement actions will be taken (Closure Order and Violation Ticket).
