PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 11020 - CCLR s.13(2) - A licensee must ensure that the community care facility and the furniture, equipment and fixtures within it are clean and in good repair while children are in attendance.
Observation: the following areas in the building need to be repaired or replaced as appropriate, the toilet seat the finish is not intact the flooring has a chip in the linoleum
Corrective Action(s): please ensure the toilet seat and floor is repaired or replaced

STAFFING: 12370 - CCLR s.37(3)(b) - Despite the employee qualifications required by Column 4 of the table in section 1 of Schedule E [group sizes and employee to children ratios], a licensee must replace an absent employee described in Column 1 of the following table who is absent (b) for 30 consecutive days or more with a person described in Column 3 opposite that employee and the row in Column 2 labelled as "≥ 30 days".
Observation: manager is absent and has been for over 30 consecutive days there are two staff who have been assigned manager status and duties however this information has not been done thru CCFL
Corrective Action(s): please ensure that CCFL is notified of the changes to the manager position

RECORDS AND REPORTING: 19260 - CCLR s.58(2)(b) - The licensee must (b) review the care plan at least once each year with a parent of the child requiring extra support and any person requested by the parent.
Observation: The care plan was not reviewed annually with the parent or any person requested to by the parent.
Corrective Action(s): care plan to guide and direct staff actions there is no evidence to support the plan has been reviewed with parents yearly , Staff stated there is a new one but it is not available for review at this time, please ensure the plan is available and reviewed with the parents
