Observation: 2 trays of repackaged samosas noted in 2 door upright freezer. Samosas were in frozen state. Phone conversation with the owner revealed that the samosa is from approved source and invoices are with the owner's accountant.
Corrective Action(s): Send the copy of the invoice to your health authority for review.

Observation: Clutter noted behind the cashier, sink area, and in walk-in cooler. Large amount to dust noted around the old change dispenser along side of rodent droppings. A dead mouse noted under the 2-compartment sink.
Corrective Action(s): Clean and organize the clutter noted in the infraction. Cleaning in hard to reach areas are required. Remove droppings then clean and sanitize the effected areas. Remove the dead mouse.

This inspection done by EHO student.
Comments:
-Handsink equipped with hot and cold water, liquid soap and paper towel.
-Individually packed rodent poison noted on site but not in use. Poison should not be used near the food. Remove the poison.
-Back door closed at the time of inspection.
-Walk in cooler: 2 deg. C
-Drink coolers: 4 deg. C
-Ice cream freezers: -18 deg. C
-Up right freezer: -18 deg. C
-Bottle of bleach noted on site.

Note:
If repackaging of samosas to be continued, clean out the clutter on top of the prep table near the handsink, prepare >= 200 ppm bleach solution (1/2 tsp bleach + 1L water) to clean and sanitize.
