Observation: Low temperature dishwasher dispensed 0ppm chlorine bleach concentration at 37.9C. This is a repeat violation.
Corrective Action(s): Immediately fix or repair the dishwasher. The dishwasher must dispense min 50ppm chlorine bleach concentration to adequately sanitize food contact surfaces and utensils.
