Corrective Action(s): Please ensure the permit to operate is posted in a conspicuous location at the food service establishment where it is vieweable to customers; Correct within 2 months. Copy of health operating permit replacement form was provided in case you are unable to find your permit to operate on-site (please refer to the permit replacement form for details).

Observation: Prep. cooler was above 4 degrees C (measured at 5.7 degrees C) at the time of inspection. Manager mentioned a technician will assess and re-service it today.
Corrective Action(s): Ensure the prep. cooler is able to maintain a temperature at or below 4 degrees C; Correct today. Please contact the district Environmental Health Officer for a follow up inspection once the prep. cooler is adjusted/re-serviced to 4 degrees C (40 degrees F) or less. Continue time-tracking controls for the small amounts of cold potentially hazardous food stored inside this prep. cooler in the interim.

Observation: A small amount of debris was present inside the ice machine.
Correction: Drain the ice and clean and sanitize the interior surface in accordance with manufacturer's instructions to maintain in a clean condition; Correct today.
Comments

Follow up inspection:
-Previously affected areas were cleaned and sanitized to remove rodent droppings.
-Steel wool was available on-site. Manager mentioned a hole below the ware-washing sink was filled with steel wool and then re-sealed.
-Pest control report from Orkin was available dated in March 2022. Pest control program was in place on a monthly basis as per Manager.
-Please continue with regular monitoring for signs of recent pest activity and your integrated pest control measures.
-Ice machine was drained and cleaned and sanitized. Manager stated they drain and clean and sanitize the interior surfaces of the ice machine on a weekly basis.
-Manager with valid FOODSAFE Level 1 refresher course (expiration date: July 22, 2023) was present on shift.
Signature on this report is not required due to the COVID-19 pandemic.
