Observation: A number of cockroaches and nymphal stages of cockroaches were observed in the glue traps. More activity noticed in the dishwashing area ( under the dishwashing line and double sink).
Corrective Action(s): You must thorouglly clean and sanitize the floors, dish washing line, sinks and surrounding surfaces to to ellminate any food and grease debris every day. This has to be done every night to elliminate food sources for mice and roaches. ( Immediately)

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation: A hole noticed in the dry wall near the electical panel, This has not been patched despite repeated citations. Grease build up noticed on the window and ledge near the deep fryers.
Grease and slime build up noticed on water lines connected to the ice maker.
Corrective Action(s): You must patch or seal any holes in the drywall as this could be a potential hiding spot for cockroaches.
Seal the gap around incoming water line using steel wool to discourage entry of pests into the premises.
Grease and slime build up on any water line connections and walls/floor in the dishwashing area must be cleaned up ( By August 27, 2021).

Comments

Improvement noted in the general sanitation and organization of the premises, especially the bar and dry storage areas.
Licensed pest control company is doing weekly monitoring.
A sanitation checklist has been created and being followed to perform enhanced cleaning.

Following additional actions must be taken:
1. Ensure to keep all items off the floor to allow for easy cleaning and pest monitoring in all areas.
2 .Clean up soft drink concentrate spills from the shelf
3. Clean any mice droppings followed by through disinfecting of the area using bleach sanitizer.
4. Seal or patch any holes or gaps in the walls or ceiling.
5. Pay special attention to prevent build up of food debris/grease on cooking equipment, cook line and dishwashing areas.
