Observation:
The double doors, leading to the smoking area, do not self-close tightly as the doors do not line-up correctly. This causes the doors to be kept open, which will allow mice to enter. There also is a gap between the double doors that will allow mice and cold air to enter.
Corrective Action(s):
Immediately have the doors repaired so they fully self-close and are not kept open. Immediately install durable weather stripping between the double doors to create a seal that will prevent the entry of mice and cold air.

Comments

A follow-up inspection was conducted, the following observations were made:
- All garbage cans in the kitchen have tight fitting lids on them. The facility is sourcing lids for the blue, recycling bins.
- Found catch-all-traps placed in various areas of the kitchen.
- A binder, for staff and residents to record sightings of droppings and mice, is present at each kitchen. Abell Pest Control Company has been instructed to review the binder each visit, to determine where activity is occurring and to take appropriate action in those areas.
- Received pest control reports for 2021 year. Reviewed the reports and it appears more attention has been placed on exterior trapping/baiting and landscaping. It is recommended that more trapping and monitoring is occurring inside the facility, where activity (droppings & sightings) has been noted.
- Education of staff and residents has occurred, regarding the interior traps and to not relocate or move them.
- General sanitation level is good. However, daily cleaning under and behind equipment (stoves, fridges, freezers) and shelves should be occurring to remove food debris, and to monitor for pest signs (dropping, nesting, etc.).
- Facility Manager explained that staff will notify the Housekeeping Dept when droppings are found, and a pylon is placed in the area of the droppings. The pylon warns staff and residents, to avoid the area until the droppings have been cleaned up. Housekeeping Dept understands that mice dropping reports are a top priority, and they will respond quickly to safely remove the droppings and to sanitize the areas.
- Increased hours for cleaning has been instituted.
- Continue to forward the pest reports to our office for review
- Discussed results of the inspection with the Facility Manager, no signature obtained due to the pandemic.
