209 - Food not protected from contamination [s. 12(a)]
Observation (CORRECTED DURING INSPECTION): a bag of onion observed on the floor in dry storage. Also, mice droppings noted aroung corners of the dry storage. All food must be above floor at all times so it is protected from contamination (mice droppings/ dirt, soil on the floor)
Corrective Action(s): Place all food at least 6 inches above floor.

304 - Premises not free of pests [s. 26(a)]
Observation: Mice droppings noted in dry storage, on trolleys and in the back room. This introduces contamination to the food and food prep areas including cutting boards, counter tops and kitchen prep tables.
Corrective Action(s):
- Clean and sanitize all areas affected by mice activity. Sanitize using 1000 ppm chlorine as discussed. Ensure to use proper Personal Protective Equipments(PPE).
- Contact your pest control company and develop a pest management plan including but nut limited to set traps and other solutions to pest issue. Send your plan to your health inspector for review. Keep records of pest control company visits and paper work for review as well.
- Block all possible enterances of mice.
- Ensure no food debris and water left on floors.
- Monitor mice activity and take action if it continuies.

Observation: Chest freezer observed to have extensive damage and in a state of no repair. Internal insulation noted out of place on freezer door. This is physical contamination to food stored in freezer. Observed Bread and one bag of fries stored in this freezer.
Corrective Action(s):
Remove opened bags of food from this freezer and transfer to a good working unit.
Do not store open bags of food in this freezer. Unopened packages are ok until the unit is repalaced.

Replace the unit so that food can be safely stored.

Routine inspection

- Hand washing stations maintained.
- Sanitizer available in spray bottles. Bleach 200 -ppm detected in spray bottles.
- Line cooler measured at 3 degree C and 8-9 C at the top. Ensure to close lids while not in use so to protect inserts from grill's heat.
- 2- comp sink in front and in the back are to be maintained clean and in sanitary condition.
- Floor is to be cleaned specially behind equipment and in hard to reach areas.
