Observation (CORRECTED DURING INSPECTION): Employee eating while preparing food in kitchen.
When instructed to wash hands began washing with gloves still on.
Corrective Action(s): -Staff do not eat within premise
-Staff do not wash hands wearing gloves
-Staff must wash hands at a handsink using warm water, soap and dry using papertowels.

Observation: Premise has both rodent and cockroach activity throughout kitchen and front service area.
-regular pest control service in place, premise has not taken any corrective actions
-premise sanitation is poor, mouse droppings under shelves, in corners, cockroaches under sinks.
Corrective Action(s): -Premise sanitation must improve significantly and be maintained on a daily basis.
-Entire premise is to undergo deep cleaning: floors, corners and hard to reach areas to be restored to a pristine state.
-All areas affected by pest activity must be sanitized after cleaning with dilute bleach sanitizer.
