Observation (CORRECTED DURING INSPECTION): Ice in the bar area was stored uncovered and lots of flies were noted in the bar area. Dead flies or contamination was noted on the unprotected ice in the cold well.
Corrective Action(s): Ice was discarded by the operator during the inspection. Reminded operator to have a lid or protection over the ice due to the flies noted in this area.

Observation (CORRECTED DURING INSPECTION): Bar reach-in cooler was noted at 13.4C at the time of inspection. Three cold potentially hazardous food was noted with the following internal temperatures: Open container of 4L homo milk at 12.9C, open container of strawberry fruit puree at 13.1C and open container of peach smoothie and frozen beverage mix at 14.0C.
Corrective Action(s): All cold potentially hazardous food stored in this bar reach-in cooler was discarded by the operator.

Observation: 1. Lots of live flies noted in the bar area and by the main dishwashing area.
2. Mouse droppings noted in the following areas: Dry storage room, below the back preparation sinks, below the main dish washer and in the bar area.
Corrective Action(s): Clean, sweep and sanitize all areas noted with mouse droppings. Continue to work with your pest control company to help with the pest issues.

Observation: 1. Dark, black encrusted buildup on the floor by the shelves holding the cleaned stainless steel containers next to the dishwashing area.
2. Dust noted around the ceiling vents throughout the kitchen.
Corrective Action(s): 1. Clean and scrubbed this area to remove buildup. 2. Clean the dust noted around the ceiling vents.

Observation: Bar reach-in cooler was noted at 13.4C at the time of inspection. All cold potentially hazardous food was removed from this cooler.
Corrective Action(s): Operator will be calling in a technician to look at this cooler. Do not store any cold potentially hazardous food in this unit until it is able to maintain at 4C or less.

Observation: 1. Cracked/ missing part of the floor tile below the upright freezer beside the ice machine.
2. Rust noted on the shelves in the walk-in cooler.
3. Missing sealant noted behind the 3-compartment sink in the main dishwashing area.
Corrective Action(s): All surfaces on the premises must be smooth, easily cleanable and imprevious to moisture. Fix the abovementioned areas to allow for effective cleaning and sanitation.

Routine inspection conducted today (restaurant was not open to the public for service at the time of inspection):
Handwash stations accessible, equipped with hot and cold running water, liquid soap and paper towel.
All refrigeration units (except for the unit noted in the bar) were at 4C or below.
All freezer units were at -18C or below.
Proper thawing observed in the reach-in cooler.
Hot hold units were holding food over 60C.
Food in general are stored protected and in the proper storage hierarchy.
Excellent sanitary condition noted in the interior of the ice machine, fume hood and deep fryers.
72.3C noted at the plate level during the rinse cycle for the high temperature dish machine.
50 PPM iodine residual detected from the glass dish washer.
