Walk-in cooler - some containers observed with missing or undersized lids, leaving food exposed.
Corrective Action(s): Food items / food containers when stored are to be properly covered to protect from contamination using fitted lids or single use plastic wrap / metal foil.

Observation: Storage room - adjacent to walk-in freezer, corner wall seem has unsealed hole from previous pest activity.
Corrective Action(s): Close hole with steel wool or cover with metal flashing to pest proof location.

Observation: Kitchen floor - under prep counter, along pass through wall, food and debris collecting.

Walk-in cooler door handles - surrounding surface has debris collecting.

Prep area (across from dishwashing station) - lower shelving below wood prep counter has trace mouse droppings.
Corrective Action(s): All areas described are to be thoroughly cleaned to remove all build-up of food and debris from surfaces.

Areas affected by pest activity must be sanitized with dilute bleach solution (100 ppm chlorine 1 tsp bleach / 1 liter water) after cleaning.

Observation: Clear plastic bin with shattered corner and extensive cracks.

Front kitchen entrance handsink - cold water faucet is missing handle.

Wicker basket used for sauce bottle storage - surfaces do not allow for ease of cleaning.
Corrective Action(s): Discard bin as plastic fragments could easily contaminate food items.

Replace cold water faucet handle.

Discard wicker basket container.
