Observation: Dishwasher did not dispense any detectable chlorine.
Corrective Action(s): Repair dishwasher. Until it is repaired, wash dishware and equipment in the dishwasher and MANUALLY sanitize. After the wash cycle, place all equipment in a 200ppm bleach solution for 2 minutes before allowing to air dry.

Observation: At time of inspection, rat ran across the gas burners, and up the pipe to the ventilation hood. Rat droppings noted in facility. Professional pest control is in place.
Corrective Action(s):
- Clean and sanitize areas where rat droppings are found using a 1:10 bleach solution (1 cup bleach: 10 cups water)
- Seal any entrances
- Remove any clutter
- Ensure all foods are in pest proof containers
- Wash utensils right BEFORE using it to ensure that it has not be contaminated
- Forward pest reports to the undersigned
