Observation: *REPEAT* (Since March 2023) Droppings observed throughout facility:
- Under shelving nearest meat department.
- Behind ice machine
- Behind large grain container storage (for mill)

2. 2 live captures (mouse) observed at time of inspection, behind large grain container, in corner nearest office, stuck on glue trap. Staff 'removed' glue trap.

3. Potential Indian Meal Moth observed in mill room. Staff 'caught' and discarded.
Corrective Action(s): 1-3: Notify professional pest control company regarding activity observed during inspection. Follow instructions to remove pest issue.

Observation: 1. Cutting board of back ware house heavily scored and soiled. Board appeard dark brown-black, with dark green growth.
2. All knives and magnetic knife strip heavily soiled during inspection.
3. Some knives heavily damaged.
Corrective Action(s): 1. Resurface, replace, or repair so surface is smooth, easily cleanable and in good working condition.
