Observation: Sanitation: Food/debris noted throughout facility. Top of equipment, storage shelves, behind/under equipment, wall spills, etc. Janitorial/electrical storage room unorganized - water leak? Play structure landing zones: dirt accumulation noted.
Corrective Action(s): Thorough cleaning of facility required. Arrange items, clean and organize. Ensure cleaning schedules are being followed for all areas.

- Temperature Control: Monitoring audits reviewed. Temperature gauge inside upright refrigeration unit (prep area) broken. Remove. Previously noted.
- Mechanical Dishwasher: Low Temp (chlorine) = Ok. Ensure all food contact surfaces (tongs, spatulas) are being washed and disinfected after use or on a timely manner. All items are to be stored in a sanitary manner in order to prevent post contamination.
- Wiping cloth storage: Limit the amount of cloths being used at any time.
- Food Storage: Storage container shelving discussed.
- Sanitary Facilities: Towel dispenser required to be permanently mounted at service handsink.
- Chemical Storage: Ok.
- Staff Hygiene: Discussed.
- Sanitation: Refer to Violation Notes.
- Pest Control: An active Pest Control Program is in place. Records reviewed. Sanitation is part of an effective pest control program.
- Garbage, Recycling and Composting Disposal: All items are to be kept stored in covered, rodent proof containers.
- Structurally: All surfaces are to be kept in good repair.
