Follow up inspection conducted:
Wooden shelf has been replaced; new shelf is smooth, impermeable to moisture, and easy to clean
Tray with bucket of utensils on top was clean (on top of the ice box)
Both sanitizer rag buckets (one in the servery area and one in the cooking area) were at 200 ppm QUATS. There was a reminder sign posted to change solution every 2 hours. Great
Did not observe dirty dishes on the 'clean side" of the dishwasher. Manager has indicated that any heavily soiled utensils/dishes (such as the hash boats) are to be pre-soaked in water/soap before being run through the dishwasher.
