The following areas were reviewed:

Observed Violations
RECORDS AND REPORTING: 19100 - CCLR s.57(2)(d) - A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner and emergency contact.
Observation: Upon review of the children's files and emergency consent cards the writer noted that two files were missing the name and number of a medical practitioner.
Corrective Action(s): Please have the family provide the Licensee with their child's medical practitioner's name and contact information. If the child doesn't have a medical practitioner the family may use the name of the medical clinic they use or alternately they may just write in "none" where applicable on their child's form.
Date to be Corrected: 28 October 2016
