Observation (CORRECTED DURING INSPECTION): Multiple violations:
- Liquid soap missing from front hand sink dispensing unit
- Single use paper towels missing from front hand sink dispensing unit
- Liquid soap and liquid soap wall mount dispensing unit missing from back hand sink..
CORRECTED DURING INSPECTION - two liquid soap dispensing units were pulled from the shelves and placed at the hand sinks.

Observation: Facility has not had a final approval for construction changes, equipment changes and operational changes. Floorplan still outstanding. Requires final review by FHA and possibly City of Burnaby. Final site inspection for new equipment by FHA has not occurred.

Observation: Sink plugs require replacement. The new sink plugs are too large to fit existing two compartment sink drainage holes and old sink plugs are dirty and one was found on the floor during the inspection.

Comments

The inspection determined that the new menu and food service has been initiated withou FHA approval. The following items discussed with Manager via the telephone.
- Finalized floorplans are still outstanding and are required to be submitted to FHA for approval.
- Facility must ensure a copy of floorplans have been submitted to the City of Burnaby for review and approval.
- Additionally, the City of Burnaby may require a physical inspection of all plumbing changes at the site.
- FHA requires an initial inspection of the facility to ensure all physical equipment is operational and meets temperature/operational standards

COMMENTS
- No evidence of pests
- Pest control program in place
- Coolers achieving temperatures of less than or equal to 4 degrees Celsius
- Freezers achieving temperatures of less than or equal to -18 degrees Celsius
- Sanitizer solution from dispensing unit at concentrations of greater than or equal to 200ppm QUAT
- Staff Personal items kept separate from kitchen equipment
- FoodSafe Certified staff present during inspection
