Corrective Action(s): Continue to monitor and contact pest control company to seal off all entry points. Remove droppings and sanitize area.

Comments

The following items have been observed:
-->Purple rice in rice cooker is measured at 61.2oC
-->All left over is discarded daily
-->Bleach sanitizer is filled in sanitizing sink
-->Bleach sanitizer is also noted in sanitizer bucket
-->At least 100ppm chlorine is detected in sanitizer
-->Handwashing stations are clear of items and accessible
-->Foods in open cans have been transferred to food grade containers
-->In-use utensils are stored in containers with ice
-->Window is closed in washroom - ensure window is also closed if screen is not all intact
-->Garbage bag on wall in back hallway has been removed - remove dust on that fan
-->No scoop or food preparation is observed at the back table, only rice cookers are noted

Note:
1. Downstairs storage should be organized at all times.
2. As mentioned in previous reports, carpet in storage room downstairs should be replaced with a material that is durable, smooth, easily cleaned, and impervious to moisture.
3. A big stainless steel pot is noted on top shelf at the back. No electrical stove is noted in facility. Remove stainless steel pot from facility.
4. Purchase sanitizer test strips to test sanitizer concentration.
