Report of Findings
Community Care Facilities Licensing received a complaint on December 4, 2023 regarding Care and Supervision.  
 
All areas of the complaint were fully investigated. Evidence was obtained through a review of records and an interview.  
 
Licensing confirmed that a parent was not advised of the facility’s transportation policy, a child was transported in a vehicle without the required insurance, and a transportation permission slip was not signed.  
 
A contravention to section 7(1)(b)(i) of the Community Care and Assisted Living Act (Standards to be maintained) was determined.  
Substantiated Contraventions: Evidence for this summary report is based on a combination of the Licensing Officer’s observations, a review of facility records and information provided by the facility staff at the time of the investigation.

Observation: A parent was not provided with a copy of the transportation policy and procedure. A Child was transported in a vehicle without the required type of insurance. A child's file did not include a transportation permission slip.
