Observation (CORRECTED DURING INSPECTION): 2 issues;
-double door upright cooler in the back of house unable to maintain contents <4C. Moved various chopped vegetables, seaweed, mayo based sauces, and tofu to the walk-in cooler.
-various chopped vegetables and tempura batter were in melted ice baths (water was not touching containers)
Corrective Action(s): Do not use the double door upright cooler until it has been repaired.
Reminder to staff and operator that the ice baths must be replenished when melted.

Observation (CORRECTED DURING INSPECTION): Ice machine found to have debris build up inside ice well.
Corrective Action(s): Remove ice and clean out ice machine. Sanitize using 200 ppm QUATS.

Observation (CORRECTED DURING INSPECTION): REPEAT OBSERVATION: observed staff member wash gloved hands
Corrective Action(s): discussed with staff and operator proper handwashing and glove use practices. Gloves are to be discarded and hands washed between tasks.

-along the back door hallway
-behind the 4 door freezer in the dishwashing and storage area
-in the corner adjacent to the double door upright cooler
Corrective Action(s): remove the rodent droppings and sanitize area. Continue to follow pest control professional's recommendations and routine cleaning

=High temperature dishwasher had a final rinse temperature of 72.9 C at the dish surface (minimum 71 C required for proper sanitizing)
=3 compartment sink with sink plugs available for manual dishwashing. Operator able to demonstrate manual warewashing method
=Sanitizer solution measured 200 ppm QUATS in spray bottles
=General sanitation has improved. continue with routine cleaning procedures
=Staff hygiene satisfactory at the time of inspection; staff FOODSAFE certification verified to be within expiry date.
