Introduction

An unscheduled routine inspection was completed to assess compliance with the Community Care and Assisted Living Act (CCALA) and the Residential Care Regulation (RCR) and the relevant Director of Licensing Standards of Practice (DOLSOP). Evidence for this report was based on the Licensing Officer’s observations, review of the facility records, and information provided by the facility staff at the time of inspection.

Care systems reviewed during inspections include the following: Licensing, Physical Facility, Staffing, Policies and Procedures, Care and Supervision, Hygiene and Communicable Disease Control, Medication, Nutrition and Food Services, Program, Records & Reporting.

As part of the routine inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection and a 3 year “historical” review of the facility’s compliance and operation.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31760 - RCR s.35(1)(c) - A licensee must provide the following appropriately furnished and equipped areas: (c) secure, safe and adequate storage areas for cleaning agents, chemical products and other hazardous materials.
Observation: Cabinets for chemical storage were found unlocked in two units of the facility. [This is a repeat contravention MMON-CC4TRT]
Corrective Action(s): Please ensure that chemicals and any other hazardous materials are securely stored as required.

CARE AND/OR SUPERVISION: 34690 - RCR s.81(3)(e)(iii) - A care plan must include all of the following: (e) in the case of a person in care who receives a type of care described as Long Term Care or who may be prone to falling, a fall prevention plan, which must address (iii) a plan for following up on any falls suffered by a person in care.
Observation: The care plans for five persons in care (PIC) were reviewed and it was confirmed that four of the PICs did not have a plan for follow up on any falls as part of their required fall prevention plan.
Corrective Action(s): Please ensure all PIC have as part of their care plan, a fall prevention plan which includes a plan for following up on any falls sufferer by the PIC.

HYGIENE AND COMMUNICABLE DISEASE: 35020 - RCR s.49(1) - A licensee must require all persons admitted to a community care facility to comply with the Province's immunization and tuberculosis control programs.
Observation: The records of five persons in care (PIC) were reviewed and the following was noted:
Tuberculosis screening for a PIC confirmed additional follow up was to be completed and the results documented, however there was no evidence to demonstrate the follow up had occurred as required.
Immunization screening for two PICs had not been completed in full.
Corrective Action(s): Please ensure compliance with the Province's immunization and tuberculosis control program for all PICs.

HYGIENE AND COMMUNICABLE DISEASE: 35040 - RCR s.63(1) - A licensee must ensure that all food is safely prepared, stored, served and handled.
Observation: - A half full serving pan of porridge was uncovered in the food warmer in one unit after serving had been completed. Of concern was the ability for persons in care to access the hot food and the increased opportunity for the food to be contaminated. [The licensing officer covered the food at that time]
- Homemade food for a person in care was found stored in the fridge in one unit. Of concern was no label on the food with a date it was received to ensure it would be consumed within a safe time frame to prevent a food born illness. Staff confirmed food is to be dated once opened, if removed from original packaging, or if provided from outside their food services department.
Corrective Action(s): Please ensure that all food is stored safely as required.

NUTRITION AND FOOD SERVICES: 37105 - RCR s.62(4) - A licensee who provides a type of care described as Long Term Care must display in a prominent place in each dining area the menu for each weekly period.
Observation: The weekly menus on display in each dining area did not include information regarding the required two snacks. The menus developed must include at least two nutritious snacks, consisting of at least two food groups.
Corrective Action(s): Please ensure each dining area has on display the menu for each weekly period.

Comments

The contraventions included in this report were reviewed in full with the facility manager and staff on the date of inspection. The report was written off-site and provided to the facility manager and licensee contact electronically with no signature required.
The results of the risk assessment were provided at the time of inspection with a copy of the assessment provided electronically to the facility manager and licensee contact.
