Observation (CORRECTED DURING INSPECTION): A block of ice inside ice machine appear to have black debris.
Corrective Action(s): Bucket of ice discarded. Clean ice machine and check that ice is clean before use.

Observation (CORRECTED DURING INSPECTION): Wet wiping towels stored on the counter after use.

Observation (CORRECTED DURING INSPECTION): Kitchen handsink has no paper towel. Bucket of sanitizer stored inside handsink.
Corrective Action(s): Ensure all handsinks has soap, paper towel, hot & cold running water and nothing stored inside.

209 - Food not protected from contamination [s. 12(a)]
Observation: Inserts of food (e.g. veggies) were stored on the ground on walk-in cooler floor. Tray of frozen raw chicken was stored in chest freezer downstairs with no cover. A bag of food was stored on top of the raw chicken.
Corrective Action(s): Store food 6 inches off the ground, utilize the shelving units. Discard bag on top of raw chicken, cover raw chicken properly with syran wrap or food grade plastic bags.

Observation: Screen net of back door downstairs is broken and no longer intact.
Corrective Action(s): Please repair screen door downstairs by follow up inspection.
