Upgrade plumbing so that both hot and cold water is supplied so that Quat can be dispensed at approx. 75F. Unsealed wood shelving observed in storage room Corrective Action(s): Contact technician to service dispenser as discussed. Quat currenty mixed by hand and tested at 200ppm. Seal open grained wood if you wish to keep the wood shelving. Violation Score: 15

Quat dispenser only dispensing at 100ppm Quat. Correction: Until Quat dispenser has been repaired, mix Quat by hand as discussed.
