304 - Premises not free of pests [s. 26(a)]
Observation:
There is a mice infestation in the premises. Droppings found on the floor along the base of walls and on lower storage shelves, in the dry storage room; walk-in-cooler; in the cookline area; and in the back food preparation area. Mice carry disease on their bodies and in their feces.
Corrective Action(s):
The following actions must be done to eliminate the infestation:
- Block all holes and entry points found with metal mesh or metal plates, as mice can't chew through metal.
- A deep cleaning of all hard to reach areas to remove any food debris or stains. This must be done daily 3 times a week.
- Inspect all areas of the premises every morning to determine where new mice droppings are.
- Wash and sanitize (with bleach) all surfaces that mice droppings have been found. This shall be done every day.
- Remove all broken, unused, or extra equipment, containers, or dishware from the premises.
- Place all food items in sealed food containers that are made of metal, hard plastic or glass.
- Inform your staff to report/record areas where mice droppings are being seen or found, and provide this information to your pest control company so they can take appropriate action in high activity areas.

305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
Observation:
1) Found two holes in walls (dry storage room and beneath salad assembly cooler), which are entry points for mice.
2) Food debris and stains found on walls and floors, behind and beneath equipment and tables. The food will attract mice and provide a food source.
3) Unused, extra or broken equipment, dishware, and containers are being stored on-site. These items have not been cleaned or moved in a long time. Mice will live and hide in and around these items.
Corrective Action(s):
1) Block the holes with metal plates or wire mesh to exclude the mice. Consult with your pest control company about what other measures to take to eliminate the mice.

2) Do a deep clean from floor to ceiling, and key on hard to reach areas such as all floors and walls 3 feet above the floor. Pull tables and equipment away from walls to access these areas behind them to clean.
**Correction date: Feb. 21/22
3) Purge and remove all unused, extra or borketn equipment, dishware and/or containers from the premises. This will eliminate harborage areas for mice.

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation:
1) The back food preparation sink has a missing hot water tap, and a broken cold water tap. This makes it difficult and unsafe for staff to properly use the faucet food preparation.
2) The upright cooler storing the desert pies, cut fruit, and veggies, has a large gap between the sliding doors, which cold air is escaping. As well, the door handles are broken off making it difficult to open and tightly close the doors. The food is not being protected from contamination and temperature fluctuations.
Corrective Action(s):
1) Repair or replace the taps on the back food preparation sink with new taps so staff can properly turn on & off the water.

2) Replace the upright cooler with a new one that can maintain 4C at all times, and is in good working condition.
