Attempt to conduct the routine inspection on May 26, 2016;

The premise is closed due to a fire. All windows is covered by a paper. A back door is open and a restoration company is on site/working.
Talk to a restoration staff on site. They cannot provide any information (and claim that an owner on site).

Talk to the owner of the pub.
The premise is burned on April 24-25, 2016.
Fire starts from the back kitchen door/some electrical issue.
The owner claims that plastic is melted and heavily contaminated the premise by black smoke.
Fire sprinkler systems went on at the time of fire and damaged the premise and equipment.

The Health Department is/was not notified.
The owner claims that all food has been discarded.

A floor plan(s) or application for renovation is not submitted to our office.
The operator claims that "every thing is going to be the same only some equipment will be replaced for new".
