1. Beside/behind counter under window in Aspen room - open side has droppings and 2 mouse traps
2. Under shelves in dry storage room in basement
Corrective Action(s): Ensure all droppings are cleaned up and areas are sanitized. Find access points and seal them. Control pest activity in facility. Train staff to monitor for any pest activity and report to manager and or pest technician.

Observation: 1. HADDON -RS Special cleaner being used in Willow servery as dining room table and food contact surface cleaner. This product appears to be used in the main kitchen as a floor cleaner. HADDON Pan Dazzle is used as all purpose cleaner. Dispenser labeled as cleaner
2. Cleaning (green) and sanitizing (red) buckets noted through out facility, some with labels and other not labeled. Some labeled cleaning buckets have EcoLabs orange cleaning product labels.
Corrective Action(s): 1. Ensure all chemical cleaners and sanitizers are being used for their intended purpose. The use of chemicals must be standardized through out facility and staff must be trained on each products use. Replace the floor cleaner for all purpose cleaner.
2. Ensure all chemical containers are properly labeled with its contents as not to confuse staff of the product and its intended use. Re-label or label all chemical containers.
