Observation (CORRECTED DURING INSPECTION):
- Fishballs stored in room temperature measured at 22C. Staff stated it has been out for 10 minutes from the hot holding unit.
Corrective Action(s):
- The volume of the fishballs would not allow temperature drop from 60C to 22C within only 10 minutes. The length of time the fishballs spent out in the room temperature cannot be determined. Product discarded.

Observation (CORRECTED DURING INSPECTION):
- Mechanical dishwasher not in use. Dishwasher is not working.
Corrective Action(s):
- Ensure to follow manual dishwashing procedure to clean, rinse and sanitize food equipment/dishes/utensils etc. Drain plug and bleach is available. Premises serve all food items in single use disposables only.

Observation:
- Live cockroaches caught in the insect monitor trap placed next to the ice machine.
- Dead cockroackes observed in the lower end wall of the ice machine area.
Corrective Action(s):
- Replace the trap.
- Remove all dead cockroackes. Clean and sanitize the area after pulling the machine out.

Observation:
- Grease build up observed on the wall (where the rice cookers are, across the 2 compartment sink.
- Old cardboards in use as liners. They are soaked with grease.
- Debris & stain build up in the undercounter cooler units.
- Front handsink is stained and observed with grime build up.
- Food debris observed on the floor (rice cooker area).
- Old empty bins present in the kitchen. Operator stated these are not it use.
Corrective Action(s):
- Clean areas noted above.
- Remove cardboards.
- Remove old empty bins.

- Mechanical dishwasher could not be drained & refilled. Operator stated that this is due to blockage in the pipe line.

- Repair the dishwasher/pipe line. It should properly operate to complete the wash, rinse, and sanitizing cycle.
