You must obtain a new business license by contacting the City of Maple Ridge. You must also obtain an application for a Health Operating permit. Please contact your environment health officer, Tim Little when these are filled out Corrective Action(s): Violation Score: 209 - Food not protected from contamination [s. 12(a)] Observation: There is a deep freezer in the staff bathroom. This is not an acceptable location to store foods because they may become contiminated. Corrective Action(s): Remove the deep freezer from the bathroom. Do not store foods in here. Violation Score: 502 - In operator’s absence, no staff on duty has FOODSAFE Level 1 or equivalent [s. 10(2)] Observation: One manager and one employee on duty and neither had Foodsafe level 1 or equivalent.
