Observation (CORRECTED DURING INSPECTION): Back hand washing sink was obstructed by pizza pans.
Corrective Action(s): Pizza pans removed. This is a designated hand washing station. Ensure that this hand washing station remains unobstructed and supplied with hot and cold running water, liquid hand soap, and single use paper towels at all times to facilitate proper hand washing.

Observation (CORRECTED DURING INSPECTION): Back door was observed to be open at time of inspection. Few flies observed in kitchen area.
Corrective Action(s): Back door was closed by staff. Ensure that back door remains closed or a proper fitting screen door is installed if back door is to remain open. Ensure that measures are taken to effectively control fly population (e.g. ensuring doors/windows are closed and removing any potential pest attractants). Contact a professional pest control operator if problem persists. Date to be corrected by: Today.

Observation: Water was observed to be dripping from the ceiling in the back area. A catch bucket is in place below the leak. No food items or food contact surfaces/equipment stored in vicinity of leaking water.
Corrective Action(s): Ensure that water is no longer dripping from the ceiling. Leaking water may potentially contaminate food and/or food contact surfaces if they are located below/near the leaking water. Date to be corrected by: August 1, 2018.

- Walk in cooler at 4 degrees C. Food is covered and stored off the ground.
- Makeline cooler at 1 to 3 degrees C (above) and 1 degree C (below)
- Hot holding case internal food temperature (pizza) at 60 degrees C or more
- Quaternary ammonium compound (quats) sanitizer available at 200 ppm (labeled spray bottles, dispenser at 3-compartment sink)
- 3-compartment sink has hot and cold running water and built-in drain plugs
- Hand washing stations are supplied with hot and cold running water, liquid hand soap, and single use paper towels.
