HAZARD RATING FOR YOUR FACILITY: Low (≤15) Total Score = 12
Critical Hazards: Total Number: 1
301 - Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
Observation: The container used to store the ice scoop in the back is dirty.
Corrective Action(s): The scoop must be stored in a clean container. Wash the container regularly.
Violation Score: 5

Non-Critical Hazards: Total Number: 3
209 - Food not protected from contamination [s. 12(a)]
Observation: 1) As per notation on the water filter in the cupboard under the dipper well, it was last changed in May, 2015. The tag on the filter in the back room indicates that it was changed in Feb., 2014. This is not acceptable!

2) The chopped up peppers are being stored inside an insert on top of a paper towel. This is not acceptable. The paper towel will soak up any water and break apart within the insert. This will contaminate the peppers.
Corrective Action(s): 1) The water filters MUST be changed regularly (usually every 4-6 months). Replace all the filters and put dates on them to indicate the next time they should be changed. Get this done by next Monday (Nov. 20th).

2) Do not store paper towels under the peppers in the insert.
Violation Score: 3

308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
Observation: The sanitizer pump at the 3-compartment sink is still not working. This was coded in the last inspection report.
Corrective Action(s): Get the pump fixed by Monday, Nov. 20th.
Violation Score: 3

311 - Premises not maintained as per approved plans [s. 6(1)(b)]
Observation: The soap dispenser has fallen off the wall AGAIN. This is a recurring issue.
Corrective Action(s): Purchase a dispenser that can be screwed to the wall, if the one with a glue back is not working. You have until next Monday (Nov. 20th) to get this done.
Violation Score: 1
