205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
Observation (CORRECTED DURING INSPECTION): Cold potentially hazardous food was stored above 4C in a refrigerator unit that has not been working since yesterday. Ambient temperature of the refrigeration unit was at 15.5C. Internal temperature of most of the cold potentially hazardous food products stored in this unit were noted around 10C. As per staff, these food items were stored either overnight or this morning (more than 4 hours).
Corrective Action(s): **Issue was corrected during the inspection**
The following food was discarded by the staff at the time of inspection (picture evidence collected): 2 containers (opened) of 4L homo milk, 4 containers of 946 mL whipping cream, 4L (opened) sour cream, 1 kg (opened bag) parmesan light cheese, 1 (cut up in 3) whole lettuce, 5 bottles of opened sauces.
DO NOT store any cold potentially hazardous food items in this refrigeration unit until it is capable of maintaining 4C or below.

302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
Observation (CORRECTED DURING INSPECTION): 25 ppm chlorine residual detected in the 3-compartment sink that was used for sanitizing dishwares.
>200 ppm chlorine residual detected in a bleach sanitizer spray bottle during the inspection.
Corrective Action(s): Purchase chlorine sanitizer test strips to allow staff to determine if they are using the correct concentration of bleach for sanitizinfg food contact surfaces and dishwares.
**Issue corrected at the time of inspection** Sanitizer was remixed during the inspection. 100 ppm chlorine residual was detected in both the sanitize compartment of the 3-compartment sink and sanitizer spray bottle.

401 - Adequate handwashing stations not available for employees [s. 21(4)]
Observation (CORRECTED DURING INSPECTION): Missing soap noted in the back of house handwash station - by the back door. As per staff, soap dispenser was broken.

Corrective Action(s): **Issue corrected at the time of inspection** Soap placed temporarily by the back of house handwash station. Fix the broken soap dispenser as all handwash stations must always be equipped with hot and cold running water, liquid soap and paper towel.

209 - Food not protected from contamination [s. 12(a)]
Observation: 1. Noted multiple containers of food in the 3-door reach-in cooler, 2 bowls (cutted green peppers and onion) in the walk-in cooler, 1 container of salt in the dry storage area and 1 bag of Durum whole wheat flour stored unprotected and opened.
2. Exposed light bulbs noted on 2 light fixtures nearby the cash register.
Corrective Action(s): 1. All food, with the exception of food that is being used for immediate services during lunch or dinner rushes, are to be stored protected/ covered either with plastic wrap or lid. This avoids potential contamination of food.
All bulk food must be stored properly in a pest proof container with lid to prevent pests from potentially getting to the food or any accidental contamination of food.
2. Purchase either the fluorescent tube protector or replaced missing shields on the light fixtures by the cash register. All exposed light bulbs are to be protected by a sleeve or shield to prevent accidental breakage of the bulbs that may contaminate food or food contact items that are stored below these unprotected fixtures.

304 - Premises not free of pests [s. 26(a)]
Observation: 1. Mouse droppings noted in the cabinet below the front handwash station and in the cabinets holding paper bags.
2. Flies observed throughout the back of house areas of the facility.
Corrective Action(s): Sweep, clean, rinse and sanitize the mouse droppings and maintain in a sanitary condition. Pests are attracted to their own fecal matters and if not cleaned when observed, they will come back and harbour in these areas. Cleaning the noted droppings will also help with monitoring pest issues.

As per staff, a new pest control company has been hired and visit the site on a bi-weekly basis. Continue working with the pest control company to deter pest issues at the facility.

Observation (CORRECTED DURING INSPECTION): Note: This is a repeated violation.
Back door was propped open during the start of the inspection.
Corrective Action(s): **Issue corrected during the inspection** Back door was closed upon request.
The back door must remain closed at all time to deter pests entering into the facility. If you wish to keep the back door open, install a screen door that will prevent pest entry when it stays open

Observation: **Note: This is a repeated violation**
Cleanliness issues noted on the wall and floor by the dishwashing area and handles and interiors of the refrigeration units.
Corrective Action(s): Clean the abovementioned areas and maintain in a sanitary condition at all times. Please follow your sanitation plan to maintain the cleanliness of the facility.

Observation: 1. Handwashing station by the walk-in cooler was not functioning at the time of inspection.
2. Front 3-door reach-in cooler was broken as of yesterday. Ambient temperature was noted to be at 15.5C during the inspection.
3. Leaking noted on the faucet of the 3-compartment sink. Missing drain plugs also noted at this 3-compartment sink.
Corrective Action(s): 1. As per staff, a technician came to look at the handwash station and was unable to fix the unit due to missing parts. Awaiting for missing parts before fixing the handwash station. Please follow-up with technician if this takes any longer as a facility must have the pre-approved required number of handwash stations functioning at all time to allow for food handlers to perform handwashing when required. Note: Two other handwash stations are close by, therefore staff are allowed to use these sinks until this noted station is fixed.

2. DO NOT store any cold potentially hazardous food in this refrigeration unit until a technician service this unit and it is able to maintain at 4C or less.

3. Purchase proper drain plugs for the 3-compartment sink. At the time of inspection, the staff were using wiping cloth to plug the sink to perform manual warewashing.
Fix the leakage noted on the 3-compartment sink.

Observation: Spray bottles noted containing chemicals with no name labels.
Corrective Action(s): Label all chemical spray bottles so that there are no question as to the content of the bottles.
This will prevent accidental chemical usage for inappropriate purposes and will help with first aide if accident is to occur.
