Observation (CORRECTED DURING INSPECTION): Observed several food handling utensils in the kitchen and at the bar sitting at room temperature in dirty water. All utensils in continuous use must be washed and sanitized every 4 hours and/or have them stored at ice water to prevent growth of pathogens
Corrective Action(s): Required to replace all dirty with clean, ice water. All utensils are to be cleaned and sanitized prior to re-use.

Observation (CORRECTED DURING INSPECTION): Observed frozen mackeral being thawed in a bucket at room temperature.

Observation: Observed rodent droppings in several areas:
1. All along the walls under coolers at sushi bar
2. In the office where all the clutter is
3. In the washrooms where a hole in the wall used to be
When asked, operator advised that the cleaning was done close to two weeks ago. Reviewed pest control report; ongoing pest issues noted on the report.
Corrective Action(s): Required operator to clean and sanitize the area for monitoring purposes. Operator has the responsibility to remove all harbourage, food and entrance points in order to remove and/or minimize pest activites in the restaurant

Observation: Observed in salmon heads in the walk-in freezer in white plastic bags. When inquired, was advised that the heads are to be used as bait when the owner goes fishing.
Corrective Action(s): Required operator to remove all products, especially items such as salmon heads not required for the restaurant, to be removed from the premises immediately.
