Observation (CORRECTED DURING INSPECTION): Pizza in hot holding case hot held below 60 degrees C. This is as per Freshslice's foodsafety plan on condition that a time management system is in place. Whiteboard indicating discard times show July 4, 2016 4-5pm. Today is July 8, 2016.
Corrective Action(s): Staff has updated the whiteboard to identify discard times. Ensure this time management system is in use at all times.

Observation (CORRECTED DURING INSPECTION):
- At the time of inspection no approved food contact surface sanitizer available for use.
- At the time of inspection 3 compartment sink not setup for manual warewashing. Observed staff members washing dishes/utensils with no sanitize step.
Corrective Action(s):
- Ensure during operation a 100ppm bleach or 200ppm quats sanitizer solution is available for use. Fantastik is not a food contact surface sanitizer.

General sanitizer formula: 1oz of regular unscented bleach to 1 gallon of water.

- Ensure all dishes/utensils are washed, rinsed, sanitized in 100ppm bleach for 2 minutes then air dryed. This is as per your sanitation plan.

Observation (CORRECTED DURING INSPECTION): New staff member caught washing hands with gloves on and not washing hands after handling money.
Corrective Action(s): Ensure hands are washed adequately at the designated hand wash station any time hands are soiled. The use of gloves does not mean hands do not have to be washed adequately.

Observation: Both staff members on duty do not have FOODSAFE level 1 or equivalent. Supervisor with FOODSAFE level 1 not onsite at time of inspection.
Corrective Action(s): Ensure in the absence of the operator at least one staff has FOODSAFE level 1 or equivalent. Send addtional staff to take FOODSAFE level 1 or equivalent if required. Correct by July 31, 2016.
