To keep PHF food (Potentially hazardous foods) at room temp , PH testing and water activity testing must be done - in the meantime keep them in cooler

Observation: No sanitizer is being used for bar area, wet wiping rags are being used??
Corrective Action(s): -Keep all wiping towels in a bucket of sanitizer (bleach solution)

Observation: -Glass washer iodine tested twice and it was low even by changing the sanitizer bucket
Corrective Action(s): -Repair to provide 12.5 to 25 ppm
-Sanitize all glasses by soaking in sanitizer for 2 minute

Observation: -The only hand washing sink in the back kitchen was blocked by large pots and some utensils were stored inside at time of visit - violation ticket will be on hold
-Staff washroom: no liquid soap provided at the handwash sink (soap in dispenser was dry and crusty??
- No paper towel and liquid soap at hand wash sink in bar area
Corrective Action(s): -Hand washing sink must be accessible at all time and no utensil washing in the sink
-Provide paper towel and liquid soap in dispenser at all hand washing sink and ensure to resupply once it is low

Observation: -Noted raw animal foods ( chicken, fish, raw eggs, meat) were stored above cooked/ready to eat products in walk in cooler, upright cooler and prep cooler
-Foods noted without proper cover
-Food containers were stacked on top of each other without lid
-Noted opened foods in open can
-Noted opened bags of dry goods, and dry goods in soft packaging without protection
Corrective Action(s): -Re arrange and store all raw animal products in lower shelf or designate on whole shelving unit for ONLY raw animal foods , store all cooked or vegetable and ready to eat products on upper shelf and away from raw products.
-Cover all foods by storing in proper container with lid
-After opening any can transfer the rest in a food grade container with proper lid
-Do not stack food containers without lid
-After opening dry goods transfer the rest in rodent proof container with proper lid

Observation: -Rodent dropping noted in dry storage room, on some shelving in kitchen, behind equipment inside glass washer chemical storage...
-Noted a dead small mice in one of the sticky traps
Corrective Action(s): -Provide a detailed pest control report by doing a full assessment of the facility to identify issues contributing to the rodent issues
-Clean all dropping and have monitoring device and traps

Observation: -There is large gap under back door screen which allows rodent entry
- Water leaking under cooking equipment
- Tiles are missing in kitchen
Corrective Action(s): -Fix the gap (Ie: install weather stripping)
-Fix the leak
-Replace missing tiles
