HAZARD RATING FOR YOUR FACILITY: High (≥30) Total Score = 63
Critical Hazards: Total Number: 3
211 - Semi critical items not adequately cleaned and disinfected
Observation (CORRECTED DURING INSPECTION): Nail clippers and nail nippers were dirty with old nail clippings and were about to be used on client. Staff stated they may have forgotten to clean and disinfect these implements. Implements were placed in CS20 solution but implements were not fully submersed into solution.
Corrective Action(s): All semi-critical implements must first be cleaned and then disinfected with a high-level disinfectant such as CS20. Implements must fully be soaked in order to achieve disinfection. CS20 solution should be labelled to indicate 14 day use period and a timer should be used. Staff were directed to disinfect implements for 20 minutes before using on client.
Recommend having a minimum of two sets of implements so that there is always one clean set ready to use.
Clean sets should be stored in a clean container.
Violation Score: 15

212 - Semi critical single use items are not discarded after use
Observation (CORRECTED DURING INSPECTION): Dirty single use nail file in equipment box that was to be used on client.
Corrective Action(s): Staff discarded. Do not re-use single use implements. All single use files in drawers that are used for "personal use" by staff were discarded as there was no way to know that these belonged to the staff.
Violation Score: 15

311 - Semi critical items not adequately cleaned and disinfected
Observation: Tweezers for lash extensions submersed in barbicide.
