Observation (CORRECTED DURING INSPECTION): Licensing noted that the manager's name is not displayed.
Corrective Action(s): Please ensuer that the Name of the Manager is displayed in a prominent place.

- 2 were missing Emergency Contacts
- 1 was missing a Doctors name and phone number
