Observation: Boxes of oil are noted storing next to white stand up freezer, on the ground.
Corrective Action(s): Ensure all products is stored at least 6 inches above ground.

Observation: 3 live cockcroaches noted in facility (2 younger cockroaches under rubber mat at dishwashing areas, and 1 adult cockroach under prep table). Dead cockroaches are also noted, beside deep fryer and under cooking line. Pest control company has been coming at least 2x a week to place bait.
Corrective Action(s): Improvement on pest activity is noted, but regular baiting is still required. Key is to keep facility clean, especially under cooking line. Operator indicated pressure washing will be done in a week.

Observation: 1. Cardboard is used to line floor under rubber mats, at dishwashing area.
2. A few chipped tiles are noted at dishwashing area, and under deep fryers.
Corrective Action(s): 1. Do not use cardboard at all in facility. All surfaces must be durable, easily cleaned, and impervious to moisture. This has been mentioned in previous inspections. Failure to comply may result in further enforcement action.
2. Operator indicated techinican will be coming this weekend to replace chipped tiles. Ensure chipped tiles are replaced for ease of cleaning, especially when facility is dealing with pest problem.
