Observation (CORRECTED DURING INSPECTION):
1. 3 sanitizer bottles and 2 sanitizer buckets present with 50-100 ppm QUATS.
2 wet cloth not fully soaked in sanitizer bucket.
3. Dough mixer is not in sanitary manner, and staff claimed it's only sanitized once at the end of the day. Staff claims it is used 2-3 times daily.

Date to Be Corrected By: Immediately.

1. Refill with 200 ppm QUATS and frequently change the sanitizer as it dissipated over time. This is to properly sanitize food contact surfaces and to prevent potential cross contamination.
2. All wet cloth must be stored fully in the sanitizer bucket to prevent microbial growth on the cloth.
3. All equipment must be washed AND sanitized every 4 hours.

- staff prepping food in the kitchen did not wash hands before and after wearing gloves, and no t asingle time of washing hands noted during the inspection.
