Observation (CORRECTED DURING INSPECTION): Facility lacking sanitizer testing strips at main three compartment sink. CORRECTED DURING INSPECTION. Items located from supply cupboard.

Pizza sauce ladle stored in water. Sauce at bottom of water. Staff instructed to wash and sanitize item. Staff washed item without sanitizing. Staff could not demonstrate how to dispense sanitizer solution. CORRECTED DURING INSPECTION. Item removed from air drying rack and properly sanitized. Health Inspector taught staff how to sanitize utensils. Signs provided to facility for staff.

Corrective Action(s): Sanitizer testing strips MUST be available for use at each station. This is a repeat violation. Instructions for day staff must be posted. Currently day staff do not clean and sanitize utensils. Day staff unable to properly demonstrate how to make sanitizer solution. Day staff need to know how to properly wash-rinse-sanitize all utensils for operation of this business. POST SIGNS.

Observation: 1) Back door to storage area propped open. Signs indicating staff must close door. Dead mouse found in area today
2) Rubber mats have food in corners and underneath
3) Floor grout is missing in some areas contributing to pooling water and accumulation of food particles.
4) Scrap piece of dough on counter. Item covered in fruit flies
5) Plugged sink in pizza pizza preparation area. Fruit flies observed to land on residual food that is unable to drain.

Corrective Action(s): All above items contribute to pest infestations. These items must be addressed immediately.
