Observation (CORRECTED DURING INSPECTION): Reviewed the Manual Wash method with a worker in the back area. Only soap and water was mentioned.
Corrective Action(s): You MUST be Manually Washing properly -as per the signs posted:

Wash --> rinse --> SANITIZE (QUAT residual >200ppm) for 2 minutes

Observation (CORRECTED DURING INSPECTION): Back washroom has single dispensing soap (dispenser is missing)
Corrective Action(s): Handsinks must be adequately stocked: paper towels dispensed, pumped soap, hot/cold water and clear for use

Observation: AGAIN observed a bare paper towel roll noticeably soiled on the ends -used for cleaning front area food contact surfaces

NOTE: this was also observed in the routine inspection on July 16, 2021
Corrective Action(s): Place the bare paper towel roll in a dispenser (spindles okay) so the ends do not get contaminated.

Final warning -increased enforcement will be required if observed again (i.e. ticket(s))
